Please return your parent signed consent form to the health office as soon as possible. If you are not participating in the shot clinic at school, be sure to have your parents schedule your shot with your doctor, or call their office and request that they fax an updated record to the school health office at 552-4701. You will NOT be able to attend the first day of school in August if you have not turned in a valid immunization certificate.
